A transition involves more than a departure time. The young person, family, current care team, travel provider and receiving program may all need different information. Adolescent Youth Services helps readers organize that work into a clear plan.

Begin with the receiving team

Confirm the destination, the receiving contact, the admission or appointment window and the documents requested. Ask the professionals involved to clarify clinical suitability and any medical requirements. Keep the travel plan connected to those decisions.

The logistics worksheet in Resources records what is confirmed, what remains open and who owns the next conversation. It also distinguishes the person coordinating travel from the professional making a clinical decision.

Prepare the people as well as the route

Include the young person’s preferences and questions where appropriate, along with the family’s practical concerns. Discuss what will happen at arrival, who will meet them and how the agreed family updates will work. Record arrangements that the people involved have actually accepted.

Connect travel and follow-through

  • Confirm the receiving contact before departure.
  • Ask which documents must arrive securely and in advance.
  • Check the practical arrival arrangements.
  • Clarify who supplies updates during the journey.
  • Identify the owner of the first step after arrival.
