Receiving-team confirmation
Start with the person who can confirm the appointment or admission. Record the destination, arrival window, named contact and information requested. Ask how changes should be reported. A web listing or informal conversation is not a confirmed intake arrangement.
Practical travel planning
Organize the route, timing, access requirements and the people supplying transport. Bring individual suitability questions to the professionals responsible for assessing them. Ask the travel provider to describe the service actually proposed rather than assume every transport works the same way.
Information and documents
Create an inventory of the documents requested by the receiving team. Identify who is authorized to supply them and the approved delivery channel. Keep clinical documents separate from the short logistics brief used by people who do not need the full record.
Family communication
Agree on who receives updates, how often updates are expected and whom to contact for a change in plans. HHS provides information about mental health privacy and family participation; individual information-sharing questions depend on the circumstances and should be confirmed with the relevant provider.
Arrival and follow-through
Confirm who meets the young person and which person owns the next step after arrival. If ongoing appointments, school coordination or family support have been discussed, record the arrangements actually made and the professional or organization responsible.
